Pear sawfly (Hoplocampa brevis). The pear sawfly overwinters as a larva in the ground. In spring, wasps crawl out of the ground after pupating. The wasp lays eggs in the flowers. The larvae eat the young fruits, which they hollow out completely.

Remove affected fruit (drill holes in the pears.).

Prevention

Control and prevent by misting with pyrethrum.
